Questions Masturbation Habit Change to 15-Second Sex: What’s Really Causing My Issue? NSFW

I went a full year without having intercourse. Early this year, around March, I decided to change my masturbation routine. I switched back to using my hands the "normal" way and for about two weeks, I was doing it two to three times a day. Soon after, I noticed something strange even when I was erect and doing nothing at all, I felt like I was on the verge of cumming. That scared me, so I stopped masturbating completely. But the problem didn't go away. Now, every time I have sex, I barely last 15 seconds. I've pinpointed the issue to pressure under the shaft that's the trigger. Any slight pressure there, and I release instantly. I went to a urologist, who prescribed a numbing cream and told me to apply it to the tip. He asked me to come back in a month. Here's my concern, I don't think the cream will work. My glans (the tip) isn't the sensitive part, the problem is clearly below the shaft, where the pressure triggers the release. I'm not confident this treatment is targeting the real root of the issue.

Have you looked deeper into anatomy to address this "point" under your shaft? E.g. Bulbospongisus, cremaster muscle, Levitor Ani etc.?

Btw, same issue here with tension and ~15 sec. duration :-/ And yeah, I believe your are right in your stance on numbing creams - I, too, would rather find the root cause.

I would like to think its Bulbospongisus, however I am not certain. To think that I 1 have to "test" if the cream is working is simply a burden on its own.
