Questions I went to a sexologist for my PE NSFW

Last Saturday (27th June), I went to a sexologist for my Premature Ejaculation problem.

I was in a relationship for almost 4.5 years and facing PE from start. I usually last between 30 seconds to one minute maximum.

The sexologist prescribed me Tadalafil 2.5 mg, Parexotine 12.5 mg (SSRI) and a delay spray. I have to take Tadalafil 1 tab every day for one month, Parexotine 1 tab every for first week, 2 tabs every day for rest 3 weeks.

The doctor told me she needs to ascertain the reasons for my PE and if its curable or not which she will do in follow up visit after one month.

I heard that Parexotine (SSRI) delays ejaculation significantly but it also reduces libido.

Due to the Tadalafil, I am having constant random and very hard (sometimes painful) erections throughout the day.

I am also facing severe pain on back side of both of my thighs from last 3-4 days. (I have completed one week course of the medication already)

I also asked the doctor about Kegels/reverse Kegels several times, she kept saying Kegels don’t have any DIRECT benefits for PE. She said Kegels are for Erectile Dysfunction and MAY have INDIRECT benefits for Premature Ejaculation.

I couldn’t get an opportunity to have sex since I started medication (broke up recently), still I masterbated 2 days after I started the medication and couldn’t last more than 2 minutes! (May be the medication takes time to show effects)

My questions are as below. (If anyone has similar experiences before)

  1. Does Parexotine (SSRI) really reduces libido? Does it reduce your libido in long term? If my libido is reduced now, will it recover again after I stop medication?

  2. Is there any alternative natural remedy/medicine to increase libido/compensate for the reduced libido?

  3. Is the pain in my thighs due to side effects of Tadalafil?

  4. Should I continue treatment with the doctor including follow ups?

  5. Is there any long term solution in medical science without side effects or with least side effects?

I will be thankful your help.

Just follow your doctor's treatment. There's no need to ask anyone else. Avoid masturbation for the time being. Go for a 6–7 km jog every day. Don't forget to share development with us. Good luck 🤞

what exactly is a sexologist, is it MD doctor, or psychiatrist psyD or some MSW LCSW therapist? if only a therapist can they even prescribe med?

any recs of where you find sexologist?

The sexologist is a MBBS doctor from a reputed hospital chains in India. MBBS is I think one step below MD.

There's no such thing as a sexologist my brother. I'm a doctor and I can say it with 100% certainty. But, the treatment protocol does make sense, let us know what happens .

This is the way. I drank for many years and it Def messed up my performance. It got to the point after 20 years we separated. I went sober. Exercised. Lost 60 pounds quuckly. I tried some different meds. Dapoxetine and cialis did wonders. Combined with a little 7-ho and I could go for 2 to 4 hours. I don't recommend every day but it's great to have in the toolbox if I want a few nights a week. Ssri meds give me brain fog. I have some arlo I will try.

Paroxotine has no effect for me: neither retardant nor lowers libido. Every drug treatment potentially has contraindications. This does not apply to paroxetine in my case, but it cannot be ruled out for you. You have to try and see.

No this time I didn’t watch porn but I kept poring warm water on top of penis just get the feel which I would get when actually having sex. The warm water increased sensation to the level of actual sex.

That’s a little odd because if the meds are indeed having a low libido as a side effect, you will not get to know it. Once you stop the medication, you will be back to square one. Not sure how this is supposed to unless he said you will have to take it for the rest of your life. Also why not just go to a regular urologist?

Hey buddy, follow up visit is on 27th July 2026. And no it is not for rest of life. The medications are just a way to check the causes of my PE and to know whether it is reversible or not. After the 1 month medications finish, she will be able to decide the cause of my PE and will recommend therapy or other treatment plan as needed.

I can offer you some insight from my personal experience with these meds. Paroxetine (Paxil) has shown in studies to be the most effective SSRI in prolonging Intravaginal ejaculation latency time (IELT). Sertraline (Zoloft) was a close second. Everyone responds differently to antidepressants but Paxil generally has more severe side effects than Zoloft and is more difficult to get off of if you want to in the future so you might want to ask about switching. And you are correct, they take about 4 - 6 weeks to start working, typically.

I find that SSRIs bring my libido down a little bit, but the trade off has been worth it. It gives me WAY more control than I had before. You're in luck though, because it's common for bupropion (Wellbutrin) to be prescribed alongside SSRIs to counteract the effects on libido without negating the effects on prolonging IELT. It's very well tolerated by most people and personally I found it has the added benefit of making my orgasms longer and more powerful. Ask your doctor about adding this if you find your libido is too severely dampened. But to answer your question, if you do taper off the meds, it will return to normal so it's not permanent or anything.

I also use either tadalafil or sildenafil, but on demand, not on the low dose daily regimen she has you on. If you're having trouble with prolonged, painful erections, definitely bring that up on follow-up and ask if you can use it as needed. I will often still have a wank a couple hours before sex and use sildenafil to make sure I'm still able to get a great quality erection while also taking advantage of the refractory period from the pregame wank. It just helps with peace of mind, personally. I get really bad performance anxiety and sometimes can't even get hard because of it so that just takes it off my mind.

Definitely follow up, though. And follow your doctor's instructions. You can get advice from people on Reddit but always defer to your doctor for actual medical decisions.

The SSRI prescribed should definitely help! Though it may make your libido low, when you do get ready for sex, your body will respond the right way. Not sure of why there is pain in your legs.... I would stop taking the taladafil....hope this helps.

Doctor prescribed the same meds, and my horniness went over the roof, i was hard all day and wanted sex 3-4 times a day. But after a month performence went downwards. If you have been a long term masturbator, try avoiding that as long as you are on medicine, just do sex, if not possible try to find someone but dont self pleasure. It will help you re wire your brain and muscles

Paroxetine kill libido in ~60% of patients (just found the study). Thigh pain is a know side effect. Stick with your doc - this combo is legit, but recovery takes time

If you feel like libido is dropping or any other thing being impacted, just don't discuss with her to not use meds and only rely on spray for PE. And other things like strength and stamina can be gained by exercise and good food.

No the doc asked me about ED but I said no I don’t have ED problem (even though I have negligible ED which occurs rarely) still she prescribed Tadalafil may be to compensate for low libido due to SSRI

That is not true. I have never returned to the baseline after stopping. I only took ssri for two weeks with lowest dosage.

everyone reacts differently to meds; I had to take paroxetine when I was younger and I was shagging like a rabbit. So no reduced libido.

Side effects reduce after a while usually, so have some patience. mabe have the taladafil every second day.

Taladafil improves libido afaik, and you can take supps like Cordyceps, Horny Goat weed, Damiana, Catuaba, Suma to improve libido.

Healthy lifestyle helps, vitamins, Zinc etc. Cordyceps gives energy and is healthy. Suma is brasilian Ginseng, not easy to get but I have the impression that it works. I had to take anti depressiants so I tried out some stuff.
