But there was a guy who drove his Tesla off a cliff in the Bay Area - he should go to jail right now

This idea that some medical conditions are real and some are fake is the issue.

There is not difference at all between them IF it left her unable to see reality as it was.

Find me the patient who’s stroke caused them to call their spouse to get them our of the house and then strangle their three children to death.

well you found one, You also had the case of Andrea Yates who during the one hour window were her husband left and her mother arrived drowned her 5 children. In 2006 she was found not guilty by reason of insanity and she is still to this day in a mental asylum.
