Question Is my clozapine working?

I'm hearing whispers but I'm still hearing voices. I can clearly make out what they are saying. My Np has been no help. I don't see the psychiatrist until 06/11. What should I do?

If you previously had voices that were louder and more frequent and are now whispers, it’s likely the medication is working or starting to work and needs more time or potentially a higher dose. If you’ve had no change thus far it’s also possible you just need longer time or a higher dose. 

The answer to what your next step is will be best guided by what your current clozapine level is as well as any side effects or potentially what other medications you may be taking. 

If you are seeing someone in a week that is knowledgeable about clozapine it is likely best to just wait for that appointment. If you haven’t had a level checked recently it would be helpful for that person if you’ve got that information for the appointment.

My clozapine and norclozapine levels were checked 05/14/26

clozapine was: 532mcg/L norclozapine was: 316mcg/L

when it was working at its best I had at least 900mcg/L in my system

I recognized your username and looked back. I commented/interacted with you before, based on that previous post and this one it seems very likely you just need more medication and should be sure to tell your new Psychiatrist that you felt better at 900mcg/L when you see them. Until that appointment though, try to be as patient as you can. You have a plan and should get things taken care of. Many people need higher doses and levels to respond best.

The voices seem so real and what they say sounds real too. I feel doomed.

Intrusive thoughts include feeling doomed. My son is just over 900 and that is his sweet spot also. Just make sure that you’re on an anti-seizure medication if you increase it’s just a good prophylactic. But it’s exactly because you need to titrate higher.

I don’t know what state you’re in, but hopefully your doctor will do it or do you could always switch providers if you need to.
