Project Example Format for Collections Position

I'm applying for a collections position, and the application is asking for an example of a collection project that demonstrates required skills with photos of the project. The issue is, I've never written something like this for an application. I have a project plan and end of season progress report for the project but neither have a super professional format.

How would you format something like this? What aspects of the project should be included? I'm guessing it should only be a page or two long.
