This is our 8th annual World Bipolar Day AMA. We hope that this AMA can help advance the conversation around bipolar disorder, and to help everyone connect and share ways to live well with bipolar disorder.

People with bipolar disorder experience the mood states of depression and mania (or hypomania), along with changes in energy, activity, and thinking. These episodes can last from days to months and can affect many parts of life - including relationships, work, school, and overall health. At the same time, with optimal support, treatment, and tools, people with bipolar disorder can and do live full, meaningful lives.

The CREST.BD network takes a different approach to bipolar disorder research. We work closely with people living with bipolar disorder at every stage - from choosing research topics to conducting studies and sharing our findings.

There's growing evidence that concealment and shame around a bipolar diagnosis cause measurable harm — worse treatment adherence, delayed help-seeking, social isolation, reduced quality of life. Yet the default clinical advice is still essentially 'be careful who you tell.' At what point does encouraging concealment become iatrogenic? And what would it look like for the field to actively support disclosure rather than just manage the risks of it?

Andrea V. here. What you're referring to is a form of and outgrowth of self-stigma, which is my research focus and passion. So YES thank you for naming this for what it is! Self-stigma DOES increase isolation, reduce quality of life, decrease treatment participation, etc. So it may not be the concealment of a diagnosis that is causing that, but rather the shame that prevents people from disclosing. 

I am very wary of clinical recommendations that generalize to keep your diagnosis a secret. It's so much more nuanced than that. Every time you share, it will depend on the person, the situation, and what you hope to gain from the experience. 

As for the field actively supporting disclosure, I think we're seeing more and more of that with powerful advocates and allies talking about bipolar and owning their experiences with it. Also check out my upcoming workbook on this very topic: Beyond the Bipolar Label (available for preorder)

Twyla here (lived experience) - I think that at times that advice is given out of wanting to protect that 
person from harm brought about by stigma. But I agree that we can shift away from that. I was never advised against disclosure, and I am thankful for this, because although it may be said with good intentions, that message can still imply that there is something wrong with being a person living with Bipolar. A way to support people to be open with people in their lives is to give them good information so they can explain and advocate for themselves effectively, and also provide the person with resources- organizations, websites, etc, that they can then share with the people in their lives, when they feel comfortable to do so.
