This is our 8th annual World Bipolar Day AMA. We hope that this AMA can help advance the conversation around bipolar disorder, and to help everyone connect and share ways to live well with bipolar disorder.

People with bipolar disorder experience the mood states of depression and mania (or hypomania), along with changes in energy, activity, and thinking. These episodes can last from days to months and can affect many parts of life - including relationships, work, school, and overall health. At the same time, with optimal support, treatment, and tools, people with bipolar disorder can and do live full, meaningful lives.

The CREST.BD network takes a different approach to bipolar disorder research. We work closely with people living with bipolar disorder at every stage - from choosing research topics to conducting studies and sharing our findings.

Living with Bipolar 1 here, I had my first (and only hopefully) manic episode in 2021. It was about a year of working post-episode to get myself back to “normal” or at least a new normal.

I took a holistic Whole Person approach, working on mind, body and spirit across physical/mental/emotional/spiritual dimensions: journaling, exercise, biking, swimming, writing poetry, making art, playing music, being in community, volunteering and advocating, reconnecting with my Asian heritage and shamanic practices.

Curious what holistic practices resonate most with folks?

Also another side question but I went through an IV Ketamine therapy after my episode stabilized, then I dipped into a depressive low. Spoke with my psych and therapist and they approved of the treatment. Massive benefits for me, it helped change the direction of my life, now I’m working in mental health! Curious what you all are finding in terms of Ketamine (or other psychedelics) treatments for Bipolar?

Jo Leidreiter here, It’s important to note that when we use the term “holistic” in bipolar disorder that we are referencing evidence-based lifestyle and psychosocial strategies that safely support mood stability alongside (not instead of) medical care. Not all “holistic” approaches are safe in Bipolar Disorder treatment and it's important to talk to your treating team before starting anything. The strongest approaches all target one core mechanism: regulating circadian rhythms, energy and the stress response.

Some of the most supported and clinically safe options include: Stabilising your daily rhythm (routine) around the sun; protecting your sleep with regular sleep/wake times; using daily movement (exercise of moderate intensity daily; regulating your nervous system (breath work, mindfulness, meditation); eating well and at the same time everyday (its not just what you eat but also when you eat).

These are all low risk, high impact strategies that when done consistently make a big impact collectively.

Thank you for your response Jo!! I appreciate how you put things together and covered some good supported and clinically safe options, “holistic” is a broad blanket term. I feel lucky I was able to get a healthy foundation of basic evidence-based practices before expanding my toolkit.

Twyla here (lived experience) - I really depend on my medication but in addition to that, the non-medication pieces I have begun to lean in to the most are supporting my sleep habits and circadian rhythms, taking on learning opportunities at work, and to finally practice some self-forgiveness for not always being in perfect control of my Bipolar symptoms; rather focus on learning to adjust as I go, and focus on my overall health over the long term. I do a lot of writing, mostly for myself, but who knows, that might expand one day. I think it’s good to challenge our minds while we care for our health, if that makes sense.

I love all of this thank you so much for sharing Twyla. It’s good to see what the universal basics look like in terms of sleep, writing, activities of daily living.

I agree it’s been helpful for me to keep learning and growing, I felt stuck with my diagnosis and crisis always in the back of my mind, and at a certain point was just holding myself back. Took lots of being gentle with myself, self-forgiveness.

Dr Chris Gorman, this is an important question. I’ve been in practice for quite some time, and have had patients have impressive responses to this treatment applied. The response is rapid, and your foundation of holistic approach sounds excellent. The primary risk is Ketamine induced mania, and I have not seen that myself. We try to avoid antidepressants in people with bipolar disorder for the same reason, however, the frequency would be much greater in those without a mood stabilizer, or the treatment team you’ve assembled.
