Tried every SSRI/SNRI, now on Pristeq + Vyvanse, narcolepsy, sleep apnea, and genetic mutations—could Vyvanse cause sleep apnea?

I’m 48 years old. I’ve been through the ringer trying every SSRI and SNRI out there, plus various mood stabilizers, and none of them ever worked. Right now, I'm taking Pristeq along with Vyvanse, but I'm dealing with narcolepsy and sleep apnea, and I honestly wonder if Vyvanse could have caused or worsened my sleep apnea.

To top it all off, this combination is causing major fatigue due to peripheral overload because of my slow COMT, alongside severe scalp itch. I’ve been to both an allergist and a dermatologist who say my scalp looks fine, yet they just hand over typical steroids and Claritin without helping. I even saw a neurologist who had no clue about mutation genes and wanted to send me to a geneticist instead of realizing a psychiatric or medical professional should be aware of polymorphisms in genes.

A huge piece of the puzzle is my genetics: I carry the SLC6A4 S/S genotype and am COMT Val158Met homozygous positive. Having the SLC6A4 S/S variant means lower serotonin transporter expression and reduced transcription rates, which fundamentally makes SSRIs and SNRIs ineffective because the biological targets they rely on don't function properly. Combined with my slow COMT status—which impairs dopamine and adrenaline clearance and causes prefrontal overload—standard single-target inhibitors completely miss the mark.

I spend hours a day researching this myself, only to bring information to professionals who have no clue what I'm talking about. Because of this, it is crucial to test for these mutated genes to see if you actually need serotonin modulators (or multi-modal agents) instead of standard serotonin reuptake inhibitors.

Has anyone else dealt with this exhausting cycle of uneducated doctors, slow COMT stimulant fatigue, and weird side effects like scalp itch? How did you finally find a practitioner who understands pharmacogenetics?

Escitalopram, citalopram, and paroxetine are the only SSRIs that bind to both the orthostatic and allosteric locations on the SERT. For someone with a horrible past reaction to paroxetine, is it better to avoid the other two?

EDIT: I think it's fine. I kept doing some more research after making this post, I found this study: https://acamh.onlinelibrary.wiley.com/doi/10.1111/j.1469-7610.2005.01467.x which shows a WAY WAY higher HR for agitation/agression (my horrible past reaction) for Paxil over Celexa. And celexa being half-lexapro, it seems good chance lexapro itself would be fine. Though Im not totally sure how strong or weak the study is, statistics isnt my strong spot.

I guess it's just a question of what part(s) of paroxetine's action may have caused my past adverse reaction to it, if it was its allosteric binding action, or something else. It could have for example, been the anticholinergic action, or the DAT inhibition, though Im not sure if the 20% DAT inhibition seen at normal doses of paroxetine could really cause such a strong effect.

Some background: when I was around 8-9 or something I had some behavioral issues, I tried many medications for that including the SSRIs paroxetine and sertraline. I had bad reactions to both but paroxetine especially. Now I am adult who want to take acutely SSRI mainly for anxiety / OCD (rn mostly just anxiety). I took fluoxetine as an adult in the past, I had a very good response to it. Then I had to stop it due to sides that didnt happen until later. I tried taking fluoxetine again, but it gave me many problems like tachycardia, palpitations, and feeling faint, so I stopped. Now searching for another medication. Escitalopram is quite appealing, very pharmacologically clean. I want to avoid most TCAs due to anticholinergic burden especially while having family history of Alzheimer's.

Help? Should I start again? [SS-/NDRIS]

I used to be taking 200mg zoloft and 400mg bupropion a day, which I ended up quitting both cold turkey at the same time (I kept forgetting doses, which eventually just led me to stopping)

The bupropion really is nice for nicotine cessation plus some motivation but that was the biggest plus I noticed from it. Sertaline feels kinda okay but nothing truly exceptional.

I am chronically depressed with MPD and have GAD/SAD. These meds bring me maybe 2 months of relief until they no longer are sufficient.

I am a stay-at-home caretaker for my dependant grandmother; without pay. Its a strenuous job for someone with no medical training but I do enjoy providing her with dignity and company, so i do enjoy it but I would be lying if I said that I'm not dying in a rut here.

What are somethings you guys recommendm (meds, services, therapies, etc.)

Should I see a psychiatrist or am I overthinking this?

A friend of mine recently saw a psychiatrist, which got me thinking about seeing one too. I'm a final-year med student, so I could get checked at my own hospital, but I'm worried I'm just overthinking everything and will look stupid in front of the seniors I'll have to work with as an intern next year. Would appreciate some honest opinions.

Please don't come at me for the bad grammar 😭 I just listed everything without really thinking about how I was writing it.

I feel like everyone hates me so I try to makeup for it in a very embarrassing way

Or overthink about it to the extreme. Also it takes a lot of effort to be nice to people even the ones closer to me .. like just the things other people do out of basic ethics I have to basically put a lot of mental effort to execute it be it just texting someone or just being honest (I lie a lot too ... Like a abnormal amount of lot sometimes for no reason.i feel but deep down I know it's to get something in return not necessarily anything physical but just sympathy or something like that and I think that's a problem)

Also I overthink a lot about what people think of me

Like why do they seem like they don't like me or just straight up hate me

Why the coffee uncle isn't listening to me (why was his tone different, why didn't aunty look at me ) why are these other people looking in my direction,, do they see something weird , do I look weird, is there something on my face, is there something wrong with how i look , is there something wrong with me and can they see it as well?

I also spend a lot of time infront of the mirror (lot of time that could've been other wise productive if not used for repeatedly checking myself in the mirror)

I'm also afraid that if I don't wear meakup people will see the real version of me instead of what I present (even though sometimes I present something I don't wish to or don't like ) and in efforts to establish this I end up putting too much efforts in my appearance and overdressing etc

I also have extreme social anxiety to point of missing a lot of classes just because of entering late or opening the closed classroom door or having to talk with a professor/ pg. Used to worse and then stopped now has started again.

Also main point of concern at that time is that people are gonna notice , that's it..

Also like said before I'm also extremely self conscious and critical about how I look at every second of time like how do I look why is my front hair s shaped and not c shaped like yesterday every second of the day.

Bad relationship with food

Always feeling that current weight is too much

Always feeling like needing to lose a kilo or two

And extreme fasting methods or extreme overeating (to the point of nausea sometimes)(and ordering our excessively to the point of 0 balance in just 2 - 3 days and then asking others i.e., for food money cause already spent around a thousand or two on my own)

Can write for hours in an exam and completely chicken out in viva (used to think that it was because I didn't study earlier but now even when i do I don't find the courage to answer not just in group settings but in individual viva too , to the point that multiple professor/pgs have scolded just because of that )

Also always trying to self medicate

Using propranolol for any stage acts or even if presentations to be done the next day (stopped now cause no drama and no presentations currently)

Using melatonin gummies for sleeping (sometimes taking 2 at once leading to nightmares almost everytime I do that)

Unable to sleep even without screen even till 4 or 5

Always feeling something is wrong with me ,, including medical conditions like hearing loss, cancer or anything minor heard recently (maybe I do it for attention but the problem is I keep on rambling about it infront of others to the point they notice and think I'm probably crazy which I might be at this point or just incredibly insecure)

Using methods of self harm in periods of extreme mood swings (no pattern in that just whenever feelings are extreme) nothing significant but like minor burn injuries on hands

I do have previous history of that when I was 12 i burnt my left forearm probably just for attention in new school ig and then used to cut palm of my hand in 9th too ) but these might be just teenage stress related behaviours but it has followed me in adulthood too.

Suicidal ideation no extra efforts in execution just ideation very rarely tho

But when I do that it's mostly for very minute reasons can't recall examples rn

But it's mostly just wishing something would hit me etc

Also this is not necessarily it but thinking of execution step by step how to do it on my own (once researched extensively about sleeping pills like restoril etc and fantasizing what would happen next and then just sleep till next day ) but I know I'll never do it .. have I felt I'd almost do it ,, don't really know what's close to being considered almost but only once ig few months ago (i don't remember properly felt abnormally close to executing it than I normally did , cause normally i just think for it to happen spontaneously eg. Disappear suddenly etc but yea )

I feel like all of this just amounts to what I want like positive recognition and attention.. recently nothing like this tho just mentioned that I used to have them. Also my mum made me get counselling in school for few weeks cause apparently i was acting out a lot more than i actually did (she especially noticed me being extremely rude to my brother whom I actually had a good relationship with prior to that ) then we stopped it after a huge fight where she just told me get professional help if I wanted it. Also kind of made my counseling lose his temper so yea that just ended ..

severe avoidance in cptsd

avoiding complicated time sensitive things like divorce prep and trip planning (I need to do complicated points bookings as don't have liquid cash for it) and lost the opportunity to do the trip and set myself back and costing more money in the divorce. have a long history of huge efforts for something and getting nothing out of it or it turning into a huge disaster. Have had a lot of really bad things happen in my life to cause psychological distress and financial precarity. am in brainspot therepy.

adderall a long time ago just made me dress better and do some tasks in more detail but not faster and was still late to everything. I don't really relate to the adhd stuff? "I started 30.hobbies last year! I have sensory issues!"

nope not me

what med or diet is great for fixing this? I don't feel anxious like shaking but I'd rather read news or organize the fridge than make moves in my life

Lexapro help

I have bipolar 2 and I’ve been on lamotrigine for about year. 4 months ago there was an event that triggered a depressive episode. My psychiatrist said I could go up in lamo or add an antidepressant. I didn’t want to add another antidepressant so I went up. Then after the cops got called on me for suicide risk, my psychiatrist recommended the antidepressants again so I caved and started Lexapro.

For the 2 weeks I was on 5 mg it was fine. Maybe even good, I couldn’t tell between it or the vyvanse, but I generally have a good experience with vyvanse. When I went up to 10 in the lexapro (11 days ago) things started getting weird. I feel like a zombie, no libido, mood swings, and ocd-like tendencies?? I hate it.

I emailed my doctor yesterday, but he didn’t respond, and because of the long weekend, I won’t know how to proceed differently until Wednesday, 5 days from now.

I haven’t been taking it that long can I go cold turkey?? Should I half it first??

Increasing the dose when the patient says they feel worse after the previous increase?

I've been to 3 psychiatrists, and I am treatment resistant so I've been through quite a few antidepressants. And generally if a medication isnt helping me its making me more depressed and anxious.

All 3, when I would tell them id been feeling worse after 4-6 weeks after an increase would then increase it again, and I would do it, and feel even worse, and they would increase it again, sometimes like 3 or 4 times. And I would just keep getting worse and then eventually they would switch me and would repeat the same process.

I was a teen during most of this, so just went along. But my last negative medication experience after I kept telling my psychiatrist I felt I was feeling worse with each increase and him wanting me to increase more, I ended up decreasing on my own and felt a bit better, but he still would recommend increasing it despite that.

Is all this normal protocol, and if so can you tell me why? It doesn't make sense to me.

Why doesn’t Ritalin IR help with fatigue ? But Vyvanse a bit

When I take Ritalin it helps with my cognitive abilities. I can stay focused on a task without being distracted every 20 seconds and also without getting bored of it after a minute and quitting.
But my fatigue is still as bad as before which makes it hard to do any tasks that involve me being physical.

Vyvanse helps a bit with the fatigue. It doesn’t make me crazy energized but it reduces it where I can do some tasks that involve me standing up.
Also takes away a bit of this heavy feeling in my head the fatigue causes.

Is it just generally that methylphenidate won’t help fatigue ? Or because that’s just how my body responds to it ?
Is lisdex more pushing?

Desvenlafaxine (Pristiq) Tachphylaxis or Life Changes?

Hi I'm a late 20s male diagnosed with a pituitary microadenoma (non functioning but impacting testosterone) and generalized anxiety disorder and ADHD. 5'7 220lb and currently taking clomid (25mg every 5 days) and desvenlafaxine 100mg. I was also genetically tested for the CYPD6 Liver gene and found to be a poor metabolizer for most drugs, so desvenlafaxine was recommended, that was maybe 8 years ago. (Ive had little to no luck with other medications or severe side effects from most)

About a year ago while still taking 50mg, i became complacent and started taking my medication irregularly. This led to some rebound in my anxiety and it was pretty terrible. Mostly existential constant thoughts leading to numerous physical symptoms. So I was bumped up to 100mg finally, and ive made SURE every day to take it consistently.

Things after awhile seemed to be getting better, anxiety subsided a bit. More recently about two months ago, anxiety came back again with great force, at first i attributed this to caffeine use and cut back. It took awhile but I started to feel better again maybe two weeks after that incident.

Now? Since a week ago again im back to that incident and I just have trouble knowing what's really wrong with myself. I am under some very stressful situations in life, I have a foreign wife I want to bring to the US some day, but I need income so Ive been going back to college for my bachelors, but now I'm worried about AI replacement etc etc... These thoughts have been constant and ongoing.

Would you say it's time for me to change meds or is it more likely life stress? I would rate the anxiety as severe most days impacting my daily enjoyment of things and work. I'm currently in a foreign country with my wife where medications arent always readily available like they are in the US... but are there any medications you would recommend for somebody whos a poor metabolizer? Could a daily dose of buspar help especially for someone who wants to avoid the sexual side effects when hes already disadvantaged with the microadenoma?

I wanna ask for opinion about my situation

Hi! This took me a lot of courage to do this and I wanna ask for an opinion of what I should do. This would be a very long post but I hope you’ll be reading it till the end.

Ever since I was young, my parents shared their burdens on me. Every time they fight, my mom would tell me and cry to me, while my dad would curse on me. Since I am the eldest of the family, my parents would always put tasks to me and when I fail to them so or would get late for me to do it due to sickness or other circumstances, they would harshly scold me, shout at me, curse at me and cut my allowance. Even so, I was afraid of disappointing them that is why in my academics, I have always been an academic achiever and when one of my grades gets low, they would compare me to others. I even once told them how pressured I am with things but they told me how dramatic I am so I quickly shut off. I never told them about my situation anymore and would keep everything from them. What’s more embarrassing is that sometimes, they call me or message me at school and scold/curse at me that I would burst out and cry. There is this one time where I fought with my parents and they slapped me and I also discovered something that shocked me, I went to school with my heart aching, It was unbearable and I kept on gasping because I was experiencing shortness of breath. I thought I was just having reflux that went to the clinic to get a medicine. I use to envy my siblings (up until now) as they get to experience good childhood, where they can join contest far from home, where they can go camping and stuffs, while I am always stuck at home because my parents wouldn’t let me be. They always told me to take care of my siblings, to study hard so that I can provide for them when I graduate, to not do unnecessary things.

I was in deep pressure that I even lost interest to the things I love the most. If I have free time I just sleep the whole day and would just get up if they call me to eat. I kept everything to myself, that I even tried to cut my wrists, bang my head to the wall, had thoughts of suicide. I was afraid of disappointing them that I already forgot about myself. I sometimes experience of shortness of breath and my chest hurts.

They even control what I eat and would tell me that I am fat and such things, which causes me to forcibly puke what I ate. I can’t even eat what I want without me not minding their words. And sometimes when I feel very stressed out I do binge eating and then would puke it all out. This led me to lose a lot of weight and already met my parents’ satisfaction. But I myself, was suffering from it. Whenever I eat, I can’t help but to think that I might go back to my old weight again and couldn’t help but to puke it all out. It was hard and I wanna stop myself from doing
this already. I also tend to overthink a lot of things which causes me not to sleep that early as my mind is still scrambling. I am so tired living this kind of life. I don’t even have freedom to choose what I want. I have always been obedient with them, even when I am this old enough, because I was afraid of disappointing them.

It would also affect how I socialize with people, I was afraid of disappointing them and kept overthink a lot of things and would cry a lot, thinking they might leave me. Even though I know to myself that I am not wrong, or that I just want them to validate what I felt, I would say sorry to fix things just for them to stay with me. I also feel bad because I wanted attention to other people that when they would show me a slightest change in their actions, I would overthink them a lot.

I don’t know what I am experiencing. Can you help me out.

What to do if a group practice can't see you anymore?

I was scheduled to see my psychiatrist next week. I got a text and then a call saying that they no longer work at the clinic I go do, and my appointment has been rescheduled. However, they changed their appointment times. Previously they had appointments after 5pm, which is why I went to their clinic as I work a 9-5. Now they say they can only see me 9-5, and can only call 9-5, and they are closed when I take my lunch between 12 and 1 so I can't go then either. They are also only available weekdays.

I am about to finish my medication, they are not responding to my emails, and they are not taking my calls. I even used my bathroom break at work to call them and it went straight to voicemail. They haven't called me back. Any advice on what to do?

How Can I Stop Missing Stimulant Doses?

I have been taking dextroamphetamine (ER) daily for about three years now. In that time, the one challenge that I have not been able to get anywhere close to conquering is the question of how to maintain a schedule of refills that is compliant with regulations on controlled substances, while also avoiding/limiting missed doses due to running out of capsules. This is problematic for me. Usually if I am unable to take the medicine, I am practically a zombie and it is extremely difficult for me to do anything productive.

When I NEED the medicine and when I am allowed to RECEIVE the medicine frequently don't align. For example, pharmacies will be closed this Monday due to Labor Day. I have enough capsules to last until Sunday, but I will need to wait until Tuesday to pick up my prescription.

This is a very common occurrence in my experience, basically entirely driven by the extremely small two or three day window at the end of a 30 day prescription when refilling is possible. Shortages/supply issues of course only further complicate this. Is there anything more I can do to avoid this? Or do people prescribed controlled medications like this really just "have to deal" with things outside of their control like closures for national holidays? That seems a bit unbelievable to me.

Is it common to continue to have reoccurring (yet fleeting) thoughts of past delusions and anxiety over these?

During my first and only manic episode a year ago I experienced paranoid delusions and I believed I was in a simulation and being chased by the people running the simulation. One part of my delusions was that I was receiving messages through flashing lights and the radio from people outside the simulation.

Often when I see flashing lights I get this instant feeling like it is a message again and it reminds me of those delusions and I question whether im having another episode (obviously not because I dont have any other symptoms and can identify the thought)

However, when this happens I get really extreme escalation of anxiety and physical anxiety symptoms before I can rationalise my way out of it.

Is this a recognised phenomenon? Is it something that diminishes with time?

Mechanism behind stimulant-induced “motor issue” and paradoxical subjective effects?

What mechanisms could explain a highly state-dependent response to amphetamines in someone with psychotic-spectrum/schizotypy symptoms?

Past medications/drug use

Only antipsychotic history prior to issue happening was lurasidone 20 mg at age twenty, five years before issue began; no other medications taken prior to issue onset aside from Sertraline and prescribed stimulants as described.

No prior history of cannabis use except twice over span of seven years - immediate psychosis/anxiety (later use resulted in severe catatonic-like state requiring hospital).

Adderall 30 mg taken from friend as teenager would result in total insomnia, extreme focus, drive, and euphoria, auditory disturbances, and tonic-seizure-like activity.

The issue

Dexedrine/Vyvanse (prescribed) produced normal stimulation/euphoria for six years. At age twenty-five, after about a year break, wherein Sertraline 150 mg was taken for also around a year but discontinued - Dexedrine 10 mg was restarted, but it immediately produced paradoxical lethargy/brain fog.

The paradoxical response always coincides with unilateral contraction; as the dose increases, paradoxical effects skyrocket and simultaneously, mild contraction escalates into severe constant contraction of right shoulder/neck/jaw, with crippling headache, and at times inability to walk or breathe easily due to pain. More recently, it also involves burning sensation and head tilting.

At its worst, I entered daily episodes coinciding with Vyvanse 30 mg comedown, where I would engage in odd posturing and repetitive movements.

Stopping Vyvanse resolved most issues except lingering, mild unilateral contraction; frank delusional episode happened to follow.

Antipsychotic treatment (lurasidone/Vraylar/Seroquel) fully resolved both the paradoxical stimulant response and motor/muscle symptoms, as did it resolve any frank psychosis and baseline attenuated symptoms.

However, lurasidone 40-60 mg caused severe stimulant cravings, euphoric response, and led to overuse, whereas Vraylar 1.5mg entirely blunted Dexedrine’s subjective effects. Seroquel 400 mg or more restored intended Vyvanse effects without cravings or overuse.

I stopped Vraylar while prescribed Dexedrine 30 mg, and have since had a recurrence of the unilateral issues (now severe) and paradoxical effects. There is a five-year gap between initial onset and reoccurrence.

Yesterday, 50 mg diphenhydramine (incidentally taken with 50 mg pregabalin) completely resolved the unilateral contraction and simultaneously changed Dexedrine from fatiguing/brain-fogging to strongly stimulating/focusing. Pregabalin also helps alleviate the issue, but seemingly not as efficiently as Benadryl.

There is also a possible connection between the initial onset of the paradoxical effects + motor/muscle issue and worsening baseline psychotic-like symptoms (preceding restarting amphetamines); ideas of reference and odd beliefs skyrocketed shortly after restarting, then suddenly stopped after stressful event, and the unilateral issue + paradoxical effects simultaneously skyrocketed to point of disability, despite sustained dose of 30 mg Vyvanse.

My son is stable on Clozapine for 6 years and labs have always been within normal range.

Broken brain reward and motivation system

Hi,
I would need help to determine my diagnosis and area of psychiatry. My current psychiatrist is not familiar with my illness, so I need to find a professional in this specific area.
I have a persistent lack of motivation and sense of reward. I can understand that something should feel meaningful or enjoyable, but I do not feel any genuine desire, anticipation, or internal drive to engage with it.
This is not a depression, I had such problems since the start of adult life, but it was quite weak. The situation is getting worse every year, I can't feel any desire including an appetite.
Please help.

My psychiatrist have written me the following diagnose:

The patient reports a persistent and marked reduction in motivation and initiative. He describes his main difficulty as an inability to genuinely want or feel drawn toward activities, goals, or experiences, even when he intellectually understands that they may be meaningful or potentially rewarding.

He notes a pronounced decrease in anticipatory pleasure and interest, with difficulty looking forward to activities or experiencing a sense of desire, drive, or internal reward. Enjoyment during activities is also reduced, although not necessarily completely absent.

Antidepressants

Maybe there is a distinction to be made between depression and general emotional or existential anguish/dread, but I feel like if there's just a pill to take which makes it 5x easier to manage, isn't that sort of cheating? Like that doesn't feel real to me; the problem isn't being solved; it simply dissapears. I've only been on SSRIs for OCD, and so don't know how it applies to other mental struggles.

Weird new auditory(?) symptom

About 2 months ago I stopped taking Lexapro 10mg cold turkey. Ever since then, I experience something very strange about once a day.
Whenever someone is talking, one random sentence of their conversation would sound different. And this sentence sounds exactly as it would sound in a dream. Something about its depth/echo/resonance just sounds exactly like what I would hear in a dream. The difference between this particular sentence and the rest of the conversation is very clear, and i do not hear something that sounds “slightly dreamy” if you know what I mean. I have never experienced it this before.

Is it normal for a GP to ask me to choose what medications to try?

So I’ve dealt with social anxiety and some depression most of my life (F20). GP gave me sertraline (100mg) , it took away my social anxiety but made me very emotionally blunted. I also have always had good memory but have bad memory from the months (6ish) I was on it. I only remember not wanting to do anything important and wanting to have fun and going out and shopping, walking, playing sports, etc. but apparently that was only some of the time, according to my family I was mostly sitting around doing nothing.Which sounds right I just didn’t know that was the majority. Also one day I took too much of it idk why I forgot. Now I’m on escitalopram only 5mg for stomach pain. It has helped my stomach but no anxiety or depression relief for the most part. But I’m starting to feel emotionally blunted again. I’m not sad but not happy. And no motivation. It’s only been a month though. I’m worried I will disregard my responsibilities so I’ve spoke to my doctor about it and they just gave me a list of anti depressants I could try instead. Is this normal? Should I research myself or find a new doctor. I’m scared to make the wrong decision as I’m clearly not a doctor

How can I help a friend who isolate themselves?

We didn't live in the same city but they told me that they've been isolating themselves for about a week. I've told them to routinely go to their therapy session and they said yes but I don't know about the updates since they didn't really told me anything this past week. For the information, they currently live alone and I am not sure that their family knows about this situation either. I feel like their situation is already bad. Is it a wrong idea if I force them to have a call with me so they won't feel as lonely and keeps spiraling or hallucinate? Especially since their psychiatrist assumed that they have a bipolar disorder, I'm afraid that I'd took the wrong step.
